Start with the 'listing multiples' method rather than complex LCD formulas. Have them write out the first several multiples of each denominator until they find a match. For example, with 1/4 + 1/6, list multiples of 4 (4, 8, 12, 16...) and multiples of 6 (6, 12, 18...) until they see 12 appears in both lists.
Teach them this simple rule: for addition and subtraction, denominators must be the same (find common denominator), but for multiplication and division, work with the denominators as given (multiply straight across for multiplication, flip and multiply for division).
Convert back the other direction as a check. If 3/8 = 0.375, then 0.375 should equal 3/8 when converted back. Also, use estimation - 3/8 should be a bit less than 1/2 (0.5), so 0.375 makes sense.
For decimals to percentages, it's always 'move the decimal point two places right' (0.25 becomes 25%). For fractions to percentages, convert to decimal first, then move the decimal point. Create a visual chart showing: Fraction → Decimal → Percentage with arrows and examples.
