The most common reason is not following PEMDAS correctly. Students often work left to right instead of doing parentheses first, or they might do addition before multiplication. Have them write out each step and identify which operation should come next according to PEMDAS before calculating.
Teach them that when multiplication and division appear in the same problem, they work left to right, not multiplication first. Use the phrase 'MD and AS are teams that work left to right' to help them remember that multiplication/division and addition/subtraction are processed in order of appearance.
Start with the innermost parentheses and work outward. Have them use different colored pencils for each level of parentheses, or teach them to identify the 'deepest' parentheses first. They should completely solve the innermost grouping before moving to the next level.
Encourage them to rewrite the entire expression after each step rather than trying to do multiple operations mentally. For hard problems, they should show 4-6 separate lines of work, with each line showing the expression after completing one operation. This prevents calculation errors and helps track their progress.
